There are two example questions here: https://coderunner.org.nz/mod/quiz/view.php?id=194 The question texts include xml exports of the questions, which you can import onto your own server and inspect.
I've subscribed you to the repo "Other languages (PHP, JavaScript, SQL ...)" . I set this up at the request of another author so teachers could share questions there, but the question bank appears to be empty at present. Feel free to contribute your own questions once you've figured things out :-)
